Non -emergency ambulances which do not originate in the <br />city, regardless of the destination. <br />5.49.030 Non -Emergency Patient Transportation Permit -- Fees. Permit <br />fees for the non -emergency transportation of patients shall be set from time to <br />time by resolution of the city council. Permit fees for nonprofit ambulance <br />services may be waived by the city council if public need and necessity require it. <br />Permit fees shall be paid before a permit is issued or renewed. On revocation of <br />a permit, the permit fees shall be forfeited to the city. <br />5.49.040 Application -- Contents. Prerequisites to the issuance of a permit <br />or renewal of a permit for an applicant shall include the payment of a <br />nonrefundable application fee to be set by resolution of the city council separate <br />from the permit fee and the filing with the Fire Department of a permit <br />application in writing, under oath, and on a form to be furnished by the Fire <br />Department. <br />5.49.050 Application -- Investigation by Fire Chief. Upon receipt of a <br />completed application, the Fire Chief, shall conduct an investigation to determine <br />if the public health, safety, welfare, convenience and necessity require the granting <br />of an original, additional, or renewal permit and shall further determine if the <br />applicant meets all requirements of this chapter. Upon completion of his <br />investigation, the Fire Chief shall recommend to the city council that a permit be <br />granted or denied for the ambulance service. The city council shall hold a public <br />hearing regarding the application. No permit shall be issued by the Fire Chief <br />until the city council has determined that the public health, safety, welfare, <br />convenience and necessity require the granting of such permit for the particular <br />ambulance service. <br />5.49.060 Permit -- Issuance or denial. A. The city council may provide <br />non -emergency ambulance service through the Fire Department and/orit may <br />order the issuance of one or more permits to conduct non -emergency ambulance <br />services in the city upon finding that the public health, safety, welfare, convenience <br />and necessity require the availability of such ambulance service and that the <br />applicant meets all requirements of this chapter. <br />B. Bonding of Applicant. Before any permit is issued to any provider <br />other than the Fire Department under the provisions of this chapter, the council <br />shall require the applicant, as a condition to the issuance of the permit, to post <br />with the clerk of the council a surety bond in the sum of seven thousand five <br />hundred dollars furnished by a corporation authorized to do business in the state, <br />payable to the city. The bond shall be conditioned upon the full and faithful <br />performance by the permittee of his obligations under the applicable provision of <br />this chapter and shall be kept in full force and effect by the permittee throughout <br />the life of the permit and all renewals tbereof.
