IN THE AMOUNT OF <br />On Sunday, April 22, 2007, two medical center employees were injured when a sink hole gave <br />way as they were accessing the entrance way to the Arrowhead Regional Medical Center. While <br />the medical center lies within the City of Colton, the incident occurred along an access road on <br />the medical center property and not a City of Colton street. The cause of this incident was the <br />failure of the regional storm drainage infrastructure located on the Arrowhead Medical Center <br />property. The same design and materials were utilized in the construction of the storm drainage <br />infrastructure located at Meridian Avenue approaching Valley Boulevard. To ensure that a <br />similar circumstance did not recur along city streets staff authorized Harris Construction, the <br />lowest bidder for the 3-5 Comprehensive Storm Drain Project Phase III, to excavate and <br />determine the condition of the Meridian Avenue storm drain; this infrastructure is a part of the 3- <br />5 Storm Drain Plan. <br />ANALYSIS/DISCUSSION: <br />On June 6, 2006, Council awarded a construction agreement to Harris Construction Engineering <br />for the 3-5 Comprehensive Storm Drain Project. This agreement was amended to include three <br />change orders to adjust for unanticipated soil conditions experienced during the construction <br />phase; total compensation reflected by agreement was $3,045,997.74. Staff instructed Harris <br />Construction to establish if a similar fault existed in the storm drainage infrastructure located <br />along Meridian Avenue as that found to exist in the infrastructure which failed on the hospital <br />property. The contractor informed staff that the bulkhead for the storm drain located along <br />Meridian Avenue, designed like the one which caused the sinkhole, were both constructed with <br />wood. As the sinkhole was a result of the wooden bulkhead having rotted away, staff advised the <br />contractor to move forward and correct the design of the Meridian Avenue storm drainage <br />infrastructure. <br />
