ITEM #5 <br />CITY OF COLTON <br />AGENDA REPORT <br />FOR THE CITY COUNCIL MEETING OF NOVEMBER 3, 2009 <br />TO: HONORABLE MAYOR AND CITY COUNCIL MEMBERS <br />FROM: MARITZA TAPIA, INTERIM PUBLIC WORKS DIRECTOR <br />SUBJECT: APPROVE CHANGE ORDER NO. 1 IN THE AMOUNT OF $3500.00 <br />TO CSI ELECTRICAL CONTRACTORS, INC. AND AUTHORIZE <br />RECORDATION OF A NOTICE OF COMPLETION FOR THE <br />LIGHTED CROSSWALK AND FLASHING BEACON <br />INSTALLATION PROJECT ON MT. VERNON AND D STREET. <br />DATE: OCTOBER 22, 2009 <br />BACKGROUND: <br />On August 19, 2008, the City Council authorized staff to solicit bids for the construction of the Teen <br />Center Project. The approved project design included traffic improvements along Mt. Vernon Ave. <br />The public bidding for the Lighted Crosswalk and Flashing Beacon Installation Project was <br />advertised in local papers and trade journals beginning on September 26, 2008. The bid opening for <br />the Lighted Crosswalk and Flashing Beacon Project was conducted and CSI Electrical Contractors, <br />Inc. was the lowest responsive bidder in the amount of $45,824.00. <br />ANALYSIS/DISCUSSION: <br />On June 06, 2009, the Notice to Proceed was issued to CSI Electrical Contractors, Inc. The <br />established date of completion for the project, as set by the terms of the agreement, was August 11, <br />2009. The project has been completed satisfactorily. Staff requests Council authorize a Notice of <br />Completion (Exhibit A) be filed and recorded, and approve a final Change Order No. 1 (Exhibit B) <br />in the amount of $3500.00. The additional work requested was installation of conduit and electrical <br />connectivity. The advance warning lighted crosswalk and flashing beacon at Mt. Vernon and D <br />Street will aid traffic movements along Mt. Vernon Avenue for pedestrian and vehicular traffic. <br />ENVIRONMENTAL IMPACT: <br />The proposed public improvements are Exempt under Section 15301 (c) of the California <br />Environmental Quality Act (CEQA). A Notice of Exemption has been filed with the County of San <br />Bernardino for the Teen Center Project improvements. <br />CONFLICT OF INTEREST <br />
