ITEM #5 CITY OF COLTON AGENDA REPORT FOR THE CITY COUNCIL MEETING OF NOVEMBER 3, 2009 TO: HONORABLE MAYOR AND CITY COUNCIL MEMBERS FROM: MARITZA TAPIA, INTERIM PUBLIC WORKS DIRECTOR SUBJECT: APPROVE CHANGE ORDER NO. 1 IN THE AMOUNT OF $3500.00 TO CSI ELECTRICAL CONTRACTORS, INC. AND AUTHORIZE RECORDATION OF A NOTICE OF COMPLETION FOR THE LIGHTED CROSSWALK AND FLASHING BEACON INSTALLATION PROJECT ON MT. VERNON AND D STREET. DATE: OCTOBER 22, 2009 BACKGROUND: On August 19, 2008, the City Council authorized staff to solicit bids for the construction of the Teen Center Project. The approved project design included traffic improvements along Mt. Vernon Ave. The public bidding for the Lighted Crosswalk and Flashing Beacon Installation Project was advertised in local papers and trade journals beginning on September 26, 2008. The bid opening for the Lighted Crosswalk and Flashing Beacon Project was conducted and CSI Electrical Contractors, Inc. was the lowest responsive bidder in the amount of $45,824.00. ANALYSIS/DISCUSSION: On June 06, 2009, the Notice to Proceed was issued to CSI Electrical Contractors, Inc. The established date of completion for the project, as set by the terms of the agreement, was August 11, 2009. The project has been completed satisfactorily. Staff requests Council authorize a Notice of Completion (Exhibit A) be filed and recorded, and approve a final Change Order No. 1 (Exhibit B) in the amount of $3500.00. The additional work requested was installation of conduit and electrical connectivity. The advance warning lighted crosswalk and flashing beacon at Mt. Vernon and D Street will aid traffic movements along Mt. Vernon Avenue for pedestrian and vehicular traffic. ENVIRONMENTAL IMPACT: The proposed public improvements are Exempt under Section 15301 (c) of the California Environmental Quality Act (CEQA). A Notice of Exemption has been filed with the County of San Bernardino for the Teen Center Project improvements. CONFLICT OF INTEREST
