Public Utilities 21881 Barton Road Grand Terrace, CA 92313 RE: Notice to Extend Contract with Clinical Laboratory of San Bernardino, Inc. Sampling and Analysis Services Dear Mr. Bob Glaubig: As you know, the City of Colton's existing contract with Clinical Laboratory of San Bernardino, Inc. to provide the professional laboratory sampling and analysis consulting services expired on April 30, 2010. Under the terms of the existing contract, a second year may be awarded to Clinical Laboratory of San Bernardino Inc. at theitC— y s_c iscr�n. The City may implement this provision of the contract by providing written notice to Clinical Laboratory of San Bernardino, Inc. on or before April 30, 2010. On April 21, 2009, the City Council, acting as the Colton Utility Authority, agreed in open session to extend the City's existing contract with Clinical Laboratory of San Bernardino, Inc. for Fiscal Year 2010-2011. Pursuant to Section 3.1.2 of the existing contract between the City of Colton and Clinical Laboratory of San Bernardino, Inc., this letter serves as the City's written notice to Clinical Laboratory of San Bernardino, Inc. that the term of the existing contract is being extended through Fiscal Year 2010-2011 (May 1, 2010 to April 30, 2011), All existing provisions of the contract shall remain in full force and effect throughout this extended term. If you agree with the terms of this notice to extend the existing contract, please indicate by signing and dating where indicated below. An original, executed coy of this/letter is enclosed for your records.
