COLTON <br />April 30, 2010 <br />Bob Glaubig <br />Laboratory Director <br />Clinical Laboratory of San Bernardino, Inc. <br />Public Utilities <br />21881 Barton Road <br />Grand Terrace, CA 92313 <br />RE: Notice to Extend Contract with Clinical Laboratory of San Bernardino, Inc. <br />Sampling and Analysis Services <br />Dear Mr. Bob Glaubig: <br />As you know, the City of Colton's existing contract with Clinical Laboratory of <br />San Bernardino, Inc. to provide the professional laboratory sampling and analysis <br />consulting services expired on April 30, 2010. Under the terms of the existing <br />contract, a second year may be awarded to Clinical Laboratory of San Bernardino <br />Inc. at theitC— y s_c iscr�n. The City may implement this provision of the contract <br />by providing written notice to Clinical Laboratory of San Bernardino, Inc. on or <br />before April 30, 2010. <br />On April 21, 2009, the City Council, acting as the Colton Utility Authority, agreed <br />in open session to extend the City's existing contract with Clinical Laboratory of <br />San Bernardino, Inc. for Fiscal Year 2010-2011. Pursuant to Section 3.1.2 of the <br />existing contract between the City of Colton and Clinical Laboratory of San <br />Bernardino, Inc., this letter serves as the City's written notice to Clinical <br />Laboratory of San Bernardino, Inc. that the term of the existing contract is being <br />extended through Fiscal Year 2010-2011 (May 1, 2010 to April 30, 2011), All <br />existing provisions of the contract shall remain in full force and effect throughout <br />this extended term. If you agree with the terms of this notice to extend the existing <br />contract, please indicate by signing and dating where indicated below.
