CITY OF COLTON NEIGHBORHOOD SERVICES DEPARTMENT INDEPENDENT CONTRACTOR AGREEMENT FOR INSTRUCTORS 1. PARTIES AND DATE: The parties to this Agreement are the City of Colton ("City") and ("Instructor"). The date of this Agreement is , 1997. 2. TERMS OF AGREEMENT: 2.1 This Agreement will become effective on and will continue for the duration of the instructional class or classes described in the "Statement of Services to be Rendered" which is set forth as Exhibit "A", or until terminated as otherwise provided for herein. 2.2 This Agreement may modified or renewed by the parties upon execution of an "Amended Statement of Services to be Rendered" which is attached hereto as Exhibit "B". 3. SERVICES TO BE PERFORMED BY INSTRUCTOR: 3.1 Instructor agrees to provide instruction in 3.2 Instructor will determine the method, details, and means of providing such instruction to those students who enroll in the class or classes up to the maximum number of students allowed as provided in the Statement of Services to be Rendered attached hereto as Exhibit "A 3.3 Instructor will be responsible for all communication between students and City regarding changes in schedule, finances and other related areas. 3.4 Should a student drop from a class, Instructor will inform City immediately by notice to the Director of Neighborhood Services or his designee. Instructor agrees to maintain an accurate record of student enrollment and attendance. 3.5 Instructor agrees to conduct the course so long as the minimum number of students enroll or so long as all classes can be combined to meet the minimum number as set forth in Exhibit "A". 3.6 Instructor confirms that he or she is qualified to teach the course and that Instructor has such training and experience as set forth in Exhibit "A". 3.7 At least six weeks prior to commencement of instruction under this Agreement, Instructor agrees to enter into a City of Colton Facility Use Agreement
