You <br />will find the amount available to your jurisdiction on the attached table. <br />As a reminder, these funds are for the installation of handicapped curb -cuts (retrofit projects) and will <br />be provided on a reimbursement basis for completed projects. Further, Article 3 funds must be <br />accounted for separately and the receipt of funds will trigger an audit for the year the funds were <br />received. <br />Enclosed is an LTF Article 3 Curb -Cut Claim form for Fiscal Year 1997-98, in the event your <br />jurisdiction will be requesting a reimbursement. Please complete the TDA Article 3 Claim Form <br />(Page 2) filling out the pertinent information and the total amount claimed. The City's Financial <br />Officer should sign this page. Page 3 of the form, (Project Information Form) should identify the <br />project name (location of curb -cuts), total cost of project and amount of LTF Article 3 used for each <br />project. The project engineer should sign the certification statement on this page. <br />You will also need to obtain governing body authorization to file the claim.
