Item #21 <br />CITY OF COLTON <br />AGENDA REPORT <br />FOR THE CITY COUNCIL MEETING OF DECEMBER 7, 2004 <br />TO: HONORABLE MAYOR AND CITY COUNCIL MEMBERS <br />FROM: AMER JAKHER, P.E., CITY ENGINEER <br />SUBJECT: FINAL ACCEPTANCE/NOTICE OF COMPLETION FOR THE <br />CONSTRUCTION OF CDBG CHILDCARE MODULAR PROJECT <br />(RECHE CANYON ELEMENTARY AND WILSON ELEMENTARY) <br />—MEADOWS CONSTRUCTION SERVICES, INC. <br />DATE: NOVEMBER 24, 2004 <br />BACKGROUND: <br />On March 2, 2004, the City Council authorized the award of contract for construction of CDBG <br />Child Care Modular Units Project to Meadows Construction Services, Inc. in the amount of <br />$335,517. The project consisted of the installation of 2160 square foot modular buildings to be used <br />for childcare services at Reche Canyon and Wilson Elementary School sites. <br />The scope of work for the project includes the installation of 2160 square feet of manufactured <br />modular building with restrooms for adults and children, kitchen, office room and stock room. <br />Construction of concrete walkways and handicap ramps was also part of the construction construct. <br />ANALYSIS/DISCUSSION: <br />The contractor has completed the work and satisfied all the requirements of contract terms and <br />agreement. The project commenced on June 9, 2004 and was completed on September 15, 2004. <br />The records indicate that this project was completed within the allowable working days. <br />CONFLICT OF INTEREST: <br />Exact Business Name: Meadows Construction Services, Inc. <br />President or equal: Rebecca M.
