Colton Joint Unified School District <br />CONSULTANT SERVICES <br />This agreement made and entered into this 8th day of October, 20 04 , by and between <br />the Colton Joint Unified School District, hereinafter referred to as the "District" and <br />Dr. Adria Klein, Ph.D., Inc. , hereinafter referred to as the "Consultant": <br />(Consultant - Company) <br />Site/Location: Crestmore Elementary <br />Date(s): November 3, 17, March 9, 29, 30 <br />Time(s): Nov3, March 9 12:30 - 3:30; Nov. 17, March 29, 30 & 7:30 - 3:30 <br />Title: On-going staff development to improve Englidh Learner performan <br />ServicLMtdontracted for: Writing process / Vocabulary Development Presentations, <br />C -1 -2 -As vlsit&ekeftq� s <br />Witnesseth: <br />Whereas, the District is in need of: school assembly / performance / presentation. <br />a developmenV inservice <br />and whereas, the consultant has some expertise in performing this service, it is mutually <br />beneficial to the parties to enter into this agreement in accordance with the following terms and <br />conditions: <br />The "DISTRICT" will: <br />1. Pay the consultant for # �� performances)inservice(s at $ each, for <br />a TOTAL COST of $ 7.000 . oo <br />2. Provide a piece of equipment: <br />3. Hold harmless the CONSULTANT for any liability imposed upon him/her for damages <br />arising out of the performance of services contracted by the DISTRICT. <br />The "CONSULTANT" will: <br />1. Submit a detailed invoice, in triplicate, to the District showing the total amount owed <br />by the District for services performed. <br />2. Grant to the District all rights, privileges and claims to the programs written for and on behalf of <br />the District in fulfilling this agreement. <br />3. Certify that all services for which payment is now being claimed were rendered at <br />times other than his/her regular assigned workday for that agency. <br />4.
