With return of the <br />agreement, also furnish a check made out to Southern Pacific= Transp Co. in <br />the amount of $385.00 to cover document preparation fee of $325.00 and $HIA <br />for first year's rental. The duplicate counterpart of the agreement will be <br />returned for your records. <br />If this work will be done by an outside contractor, you must advise the <br />name and address of the contractor in order that we may process our standard <br />Right of Entry permit with them prior to commencement of work. <br />If your own forces perform this work, you must contact the following a <br />minimum of five (5) days in advance of your planned commencement of work. <br />Fax this form to: <br />1) S.F.T. Telecom will <br />advise you if underground fiber-optic facilities are involved with your <br />project and, if so, the telephone number of the cable company that you will <br />be required to contact. S.F.
