CITY OF COLTON
INTEROFFICE MEMORANDUM
TO: MARK LEWIS, City Manager
THROUGH: DAVID G. HALL, Fire Chief
FROM: BRUCE HAMP Emergency Services Coordinatorlov
g Y
RE: ITEMS FOR FEDERAL DISASTER ASSISTANCE
DATE: MARCH 10, 1992
Attached you will find three (3) documents required in our
application for federal assistance, as follows:
OF
`MooevoMr
0
iaaz
llFOR
OES Form 130, "Designation of Applicants Agent Resolution".
At briefings held jointly by the Federal Emergency Management
Agency (FEMA) and the State of California, Office of
Emergency Services (OES), it was strongly suggested that
three (3) agents be delegated. This was to prevent slowing
of procedures in the event the designated agent was not
available to sign required documents. I would suggest agents
be named as follows:
BRUCE HAMP, Emergency Services Coordinator
HELEN RAMOS, City Clerk
MARK LEWIS, City Manager
OES Form 89, "Project Application For Federal Assistance".
Those giving the briefings stated that if we object to any
part of this form, we may line that section out and submit a
letter stating the reasons for our objection. We would than
apply for approval. I submit this for your review.
Agenda Report, "Designation of Applicants Agent Resolution",
for Council Meeting, March 17, 1992.
Thank you.
