HUTTON, PUBLIC WORKS DIRECTO , <br />a <br />SUBJECT: FINAL ACCEPTANCE f NOTICE OF COMPLETION FOR <br />THE 1998 199 PHASE I PAVEMENT REHABILITATION <br />PROJECT — CONTRACTOR MATICH CORPORATION. <br />DATE: <br />BACKGROUND: <br />March 8, 1999 <br />The 1998 - 1999 Phase 1 Pavement Rehabilitation Project consisted of a 2 <br />cold plane and 2" A.C. overlay, asphalt pothole repairs and rubberized crack <br />sealing on various city wide streets. <br />The Project was approved and awarded to Matich Corporation at an estimated <br />cost of $566,664.40. During the course of the project, eight change orders were <br />necessary, total cost of project $733,330.35. A portion of these change orders <br />was made as a result of rescheduling work from phase If project area to phase I <br />Project area. <br />SUMMARY: <br />The contractor has completed the work as specified in the contract on <br />February 23, 1999. The records indicated that this project was completed within <br />the allowable working days. <br />FINANCIAL IMPACT: <br />This Project is funded by Measure I Funds and Host City Fees.
