HUTTON, PUBLIC WORKS DIRECTOR /f <br />SUBJECT: FINAL ACCEPTANCE / NOTICE OF COMPLETION FOR <br />C.D.B.G. PROJECTS #104-21905, #104-22005, & 104-23105 <br />AND JULIE / GREY — CONTRACTOR WM. KANAYAN <br />CONSTRUCTION <br />DATE: APRIL 24, 2000 <br />BACKGROUND: <br />The C.D.B.G. projects #104-21905, #104-22005, & 104-23105 and Julie / Grey <br />consist of the installation of missing curb and gutter, sidewalks, driveway <br />approaches, handicap ramps and street pavement Rehabilitation. <br />The project was approved and awarded to WM. Kanayan Construction at an <br />estimated cost of $179,053.80. <br />During the course of the project, six change orders were necessary, increasing <br />the project additional work to $52,020.72 — see exhibit A — 1 through 8 <br />SUMMARY: <br />The contractor has completed the work as specified in the contract and a final <br />inspection has been performed. The records indicate this project was completed <br />within the allowable working days. <br />Staff has prepared a Notice of Completion form for this projectpee Exhibit "B". <br />ENVIRONMEWAL nMPACr <br />None. <br />Page 1 of 2 Item #11 <br />
