Hutton, Public Works Director <br />SUBJECT: LAKE CADENA SEWER LIFT STATION REMEDIATION Date: November 22, 1991 <br />REQUEST FOR CHANGE ORDER <br />f7_y-MMO 7iNikfki <br />The Lake Cadena Sewer Lift Station Remediation Project is a capitol improvement <br />project nearing completion and funded out of the Sewer Capitol Improvement Budget <br />for fiscal year 1990/91. <br />Due to delays in manufacturer delivery of materials, revised piping requirements <br />caused by underground utility crossing that were not of public record, and <br />recommendations for modifications by the project engineer, a change order is being <br />requested to finalize the project. <br />Krieger & Stewart, Project Engineers to the remediation work, have reviewed the <br />request and recommends granting approval of a 45 day time extension and the <br />approval of $17,598.97 for additional materials and labor. <br />RECOMMENDATION <br />Staff recommends granting approval of a 45 day time extension and authorization of <br />additional funds in the amount of $17,598.97 to complete the Lake Cadena Sewer <br />Lift Station Project. <br />The requested change order reflects less than 6% of the total contract amount. <br />JCH/cm <br />CITY MANAGER CONCURRENCE: //it <br />Attachments: <br />Page i of 1 Item No. 13 <br />FILE NO.�----.
