B. An annual amount of up to $100.00 towards lenses and frames <br />for employee and $50.00 towards lenses and frames for depen- <br />dents. If dependents do not require a City -Paid vision examina- <br />tion, City pays up to $90 for lenses and frames. <br />C. Exceptions: Those employees and eligible dependents who wish <br />to continue receiving vision examinations from their own <br />Optometrists/Opthalmnlogists may do so. <br />INSURANCE PREMIUMS/INDUSTRIAL INJURY <br />The City will pay a maximum of three (3) months insurance premiums <br />for employees on leave of absence without pay due to industrial <br />injury. Payment of insurance premiums will include health, dental, <br />life and optical. <br />TERM LIFE INSURANCE <br />The City shall provide a total of $16,500 term life insurance for <br />each represented employee. <br />RETIREES' HEALTH INSURANCE PARTICIPATION <br />Individuals who shall thereafter retire (service or disability) from <br />the City employ, may, at the retiree's discretion, enroll in the City - <br />provided health insurance plan of the employee's choice. <br />All premiums required by the retiree's participation in such health <br />insurance plan shall be paid by the retiree. Participation in any <br />health insurance plan is contingent upon the health insurance carrier <br />approving the enrollment of a retired employee. <br />LONG TERM DISABILITY <br />The City shall provide to each represented employee a long term disa- <br />bility program with a minimum waiting period of 30 days or upon the <br />expiration of sick leave, whichever is the greater period of time. <br />DENTAL PLAN <br />The City shall provide dental insurance for each represented employee <br />and eligible dependents in accordance with existing City dental plans. <br />DEFERRED COMPENSATION <br />The City shall contribute 2.147% of base salary toward a deferred <br />compensation plan for each employee represented by the Association. <br />5 <br />
