SALARY <br />Effective December 27, 1986, all employees in the above -represented <br />classifications shall receive a base salary increase of four percent <br />(4%) <br />Effective December 26, 1987, all employees in the above -represented <br />classifications shall receive a base salary increase of five percent <br />(5%). <br />Effective December 24, 1988, all employees in the above -represented <br />classifications shall receive a base salary increase of five percent <br />(5%). <br />In the event that the Consumer Price Index ("CPI", 1967 Index, Urban <br />Wage Earners and Clerical Workers, Los Angeles -Long Beach -Anaheim <br />Metropolitan Area) exceeds five percent (5.0%) for the twelve month <br />period ending October 31, 1988, the salary adjustment scheduled for <br />December 24, 1988, shall be equivalent to the above -referenced CPI. <br />However, in no event shall such increase exceed six and one-half <br />percent (6.5%). <br />HEALTH INSURANCE <br />The City shall contribute a sum not to exceed $240.22 per month per <br />employee for City provided health insurance coverage. This amount <br />shall be increased on November 1, 1987, November 1, 1988, and <br />November 1, 1989, to a flat dollar amount equivalent to the cost of <br />"Family" coverage under the Health Net Medical Insurance Program, <br />Group Number 50766A. "Family" coverage is that coverage which exists <br />for more than two persons. Any increases in the City's flat dollar <br />contribution after the expiration of this memorandum of understanding <br />shall be subject to the meet and confer process. Should such Health <br />Net coverage cease to be available during the term of this memorandum <br />of understanding, the parties shall meet and confer on an equivalent <br />alternative health plan. <br />Each employee is required to maintain a minimum coverage for him/ <br />herself in a plan of their choice. In those instances where the <br />employee's medical insurance premium is less than the City's monthly <br />contribution, the difference between said dollar amounts shall be <br />deposited in a deferred compensation program on behalf of the <br />employee, or may be provided, at the request of the individual <br />employee, in the form of a bi-weekly cash disbursement. <br />TICAL PLAN <br />The City shall provide an optical plan as follows: <br />A. An annual vision examination up to $40.00 by a licensed <br />Ophthalmologist for the employee and eligible dependents. <br />4 <br />
