SALARY <br />Effective December 27, 1986, all employees in the above classifications shall <br />receive a base salary increase of four percent (48). <br />Effective December 26, 1987, all employees in the above classifications shall <br />receive a base salary increase of five percent (5%). <br />Effective December 24, 1988, all employees in the above classifications shall <br />receive a base salary increase of five percent (58). <br />In the event that the Consumer Price Index ("CPI, 1967 Index, Urban Wage <br />Earners and Clerical Workers, Los Angeles -Long Beach -Anaheim Metropolitan <br />Area) exceeds five percent (5.0%) for the twelve month period ending October <br />31, 1988, the salary adjustment scheduled for December 24, 1988, shall be <br />equivalent to the above -referenced CPI. However, in no event shall such <br />increase exceed six and one-half percent (6.5%). <br />HEALTH INSURANCE <br />The City shall contribute a sun not to exceed $240.22 per month per employee <br />for City provided health insurance coverage. This amount shall be increased <br />on November 1, 1987, November 1, 1988, and November 1, 1989, to a flat dollar <br />amount equivalent to the cost of "Family" coverage under the Health Net <br />Medical Insurance Program, Group Number 50766A. "Family" coverage is that <br />coverage which exists for more than two persons. <br />Each employee is required to maintain a minimum coverage for him/herself in a <br />plan of their choice. In those instances where the employee's medical <br />insurance premium is less than the City's monthly contribution, the difference <br />between said dollar amounts shall be deposited in a deferred compensation <br />program on behalf of the employee, or may be provided, at the request of the <br />individual employee, in the form of a bi-weekly cash disbursement. <br />OPTICAL PLAN <br />The City shall provide an optical plan as follows: <br />A. An annual vision examination up to $40.00 by a licensed Ophthalmologist <br />for the employee and eligible dependents. <br />B. An annual amount of up to $100.00 towards lenses and frames for employee <br />and $50.00 towards lenses and frames for dependents. If dependents do not <br />require a City -paid vision examination, City pays up to $90.00 for lenses and <br />frames. <br />C. Exceptions: Those employees and eligible dependents who wish to continue <br />receiving vision examinations from their own Optometrists/Ophthalmologists <br />may do so. <br />INSURANCE PREMIUMS/INDUSTRIAL INJURY <br />The City will pay a maximan of three (3) months insurance premiums for <br />employees on leave of absence without pay due to industrial injury. Payment <br />of insurance premiums will include health, dental, life and optical. <br />2 <br />
