TERM LIFE INSURANCE <br />The City shall provide all employees Term Life Insurance coverage in the <br />amount of $50,000, with the exception of the City Manager who shall receive <br />$100,000. <br />RETIREES' HEALTH INSURANCE PARTICIPATION <br />Individuals who shall thereafter retire (service or disability) from the City <br />employ, may, at the retiree's discretion, enroll in the City -provided health <br />insurance plan of the employee's choice. <br />All premiums required by the retiree's participation in such health insurance <br />plan shall be paid by the retiree. Participation in any health insurance plan <br />is contingent upon the health insurance carrier approving the enrollment of a <br />retired employee. <br />Persons serving in Executive positions as of January 1, 1987, shall be eligible <br />for retirement health insurance pursuant to Resolution 4598. Future Executives <br />(those obtaining the position after January 1, 1987) shall not be eligible for <br />retirement health insurance pursuant to Resolution 4598 (see Resolution <br />No.R-37-lattached and incorporated herein by reference as AmerAment 1). <br />IANC TERM DISABILITY <br />The City shall provide to each represented employee, with the exception of <br />Police Chief, a long term disability program with a minimum waiting period of <br />30 days or upon the expiration of sick leave, whichever is the greater period <br />Of time. <br />The City shall provide dental insurance for each Executive employee and eligible <br />dependents in accordance with existing City dental plans. <br />DEFERRED COMPENSATION <br />The City shall contribute 2.147% of base salary plus $15.00 per month toward <br />a deferred compensation plan for each employee, with the exception of the <br />Police Chief and the Fire Chief. <br />SICK LEAVE/FAMILY AWOMANCE <br />Employees shall have the option of using sick leave for attendance on family <br />members, or the employee may elect to take leave without pay for attendance <br />on family members. Family members is defined in the City's personnel rules <br />as employee's father, father-in-law, mother, mother-in-law, brother, brother- <br />in-law, sister, sister-in-law, wife, husband, child, grandparent or grandchild. <br />ACCIMILATED SICK LEAVE AT TERMINATION OF II4PLOYE'E <br />Except as otherwise herein stated, accumulated sick leave is lost when the <br />employee is terminated. In no event shall employees who have not worked for <br />the City as regular salaried employees for more than five years be entitled <br />3 <br />
