SALARY <br />III <br />IV. <br />[I7 <br />VI. <br />Effective January 1, 1992, employees in the above referenced <br />classifications shall receive a base salary as set forth in <br />the Pay and Classification Plan for the City of Colton. <br />LONGEVITY PAY <br />Employees who have been at E step for two years and have <br />received satisfactory performance evaluations for both years <br />shall receive a one time two percent longevity adjustment. <br />HEALTH INSURANCE <br />The City shall contribute an amount equal to the cost of <br />"Family" coverage under the Health Net Medical Insurance <br />Program, Group Number 50766A per month per employee for City <br />provided health insurance coverage. "Family" coverage is that <br />coverage which exists for more than two persons. <br />Each employee is required to maintain a minimum coverage for <br />him/herself in a plan of their choice. In those instances <br />where the employee's medical insurance premium is less than <br />the City's monthly contribution, the difference between said <br />dollar amounts shall be provided in the form of a bi-weekly <br />cash disbursement. <br />OPTICAL PLAN <br />The City shall provide an annual amount of up to $140.00 <br />towards a vision examination and lenses and frames for <br />employees and $90.00 towards a vision examination and lenses <br />and frames for eligible dependents. <br />INSURANCE PREMIUMS/INDUSTRIAL INJURY <br />The City will pay a maximum of three months insurance premiums <br />for employees on leave of absence without pay due to <br />WP2:EXECBENF.WP <br />
