industrial injury. Payment of insurance premiums will include <br />health, dental, life and optical. <br />VII. TERM LIFE INSURANCE <br />The City shall provide all employees term life insurance <br />coverage in the amount of $50,000 with the exception of the <br />City Manager who shall receive $100,000. <br />VIII.RETIREES' HEALTH INSURANCE PARTICIPATION <br />Persons serving in Executive Positions as of January 1, 1987, <br />shall be eligible for retirement health insurance pursuant to <br />Resolution 4598 (attached as Exhibit A and incorporated herein <br />by reference). All other Executive employees shall not be <br />eligible for retirement health insurance pursuant to <br />Resolution 4598 (See Resolution No. R-33-87, attached and <br />incorporated herein by reference as Exhibit B). They will be <br />eligible for retiree health insurance as set forth in <br />Resolution R-115-91 (attached as Exhibit C and incorporated <br />herein by reference). <br />The City will provide the retirees' health insurance to <br />retirees who move outside the Health Net or Kaiser service <br />area and enroll in a health insurance plan. The benefit will <br />be reimbursement for monthly premiums required by the City up <br />to the current Health Net premium rate. Eligible retirees <br />must show proof of insurance and of payment of monthly <br />premiums. <br />IX. LONG TERM DISABILITY <br />The City shall provide to each represented employee, with the <br />exception of the Police Chief, a long-term disability program <br />with a minimum waiting period of 30 days or upon the <br />expiration of sick leave, whichever is the greater period of <br />time. <br />X. DENTAL PLAN <br />The City agrees to furnish to each Executive employee and <br />eligible dependents in accordance with existing City dental <br />plans. <br />XI. TUITION REIMBURSEMENT <br />All Executive employees may be reimbursed up to $1,000 per <br />year, per employee for 100% of tuition costs and for 75% of <br />textbook costs. Such expenditure must enhance furtherance of <br />City or continuing educational goals. Requests for such <br />reimbursement must be approved by the Personnel Officer. <br />WP2:EXECBENF.WP 2 <br />
