provided health insurance plan of the employee's choice. Employees who <br />retire after 15 but less than 20 years of City service shall be <br />eligible for city -paid employee only medical insurance coverage, until <br />medicare age with dependent care available at employee expense. <br />Employees who retire after having served a minimum of 20 years with the <br />City shall have their, and their spouse's, premiums paid for by the <br />City up to medicare age. After the retiree reaches medicare age, <br />he/she can maintain health insurance with the City, but the retiree <br />must pay his/her, and his/her spouse's, premiums. If the retiree is <br />ineligible for medicare benefits, the City will continue to pay the <br />premiums, as long as the employee remains insurable. For all other <br />employees, all premiums required by their, and their spouse's, <br />participation in such health insurance plan shall be paid by the <br />employee. Participation in any health insurance plan is contingent <br />upon the health insurance carrier approving the enrollment of a retired <br />employee or spouse. <br />The City will provide the retiree's health insurance benefit <br />described above to retirees who move outside the Health Net or Kaiser <br />service area and enroll in a health insurance plan. The benefit will <br />be reimbursement for monthly premiums by the City up to the current <br />Health Net premium rate. Eligible retirees must show proof of <br />insurance and of payment of monthly premiums. <br />Section 3: <br />Dental Plan <br />The City shall provide dental insurance for each represented full- <br />time employee and eligible dependents in accordance with existing City <br />dental plans. <br />Section 4: Optical Plan <br />The City shall provide an annual amount of up to $140 towards a vision <br />examination and lenses and frames for employee and $90 towards a vision <br />examination and lenses and frames for eligible dependents. <br />Section 5: Term Life Insurance <br />The City shall provide a total of $50,000 term life insurance for <br />each represented full-time employee effective the first month following <br />Council adoption of this MOU. <br />Section 6: Long Term Disability <br />The City shall provide to each represented full-time employee a long <br />term disability program. The terms of the plan shall be more fully set <br />forth in the plan documents; however, effective the first of the month <br />following City Council adoption of this MOU, it shall provide for up to <br />five years of coverage at 66-2/3% of the first $6000 of the employee's <br />base salary, reduced by any deductible benefits.
