EXHIBIT A <br />XVII. OPTICAL REIMBURSEMENT: The amount of optical reimbursement will be <br />calculated at the biannual rate of $280 for employees and $180 for eligible <br />dependents. In the event an employee or dependent elects laser eye surgery in <br />lieu of using the optical reimbursement for exams, eye glasses or contact lenses, <br />the employee may either bank unused optical reimbursement funds in anticipation <br />of future surgery, to be paid out upon receipt of invoice for completed surgery; or <br />may elect maximum reimbursement on a biannual basis until the total amount of <br />the laser surgery is reimbursed over several biannual reimbursement window <br />periods. <br />XVIII. PERS 4TH LEVEL SURVIVOR BENEFIT: The City will provide CAPERS 41h <br />Level Survivor benefits to all General Unit employees. This benefit will be made <br />available upon completion of City and Ca1PERS processing requirements revising <br />CalPERS Contract Amendment to include Section 21574 for Miscellaneous <br />Employees. <br />XIX. VACATION ACCRUAL: Language in Article IV, Section 1,B which refers to <br />employees who exceed the maximum accumulation of vacation hours will be <br />removed as it no longer applies. <br />XX. SICK LEAVE: Language in Article IV, Section 3 will be revised to remove use <br />of sick leave for family attendance for brother-in-law and sister-in-law and will <br />add step children and step parents. <br />XXI. SICK LEAVE BUY BACK: The City agrees to buy back 40 hours per year of <br />sick leave at 110% of the regular rate of pay per fiscal year. Employees must <br />have used less than 51% of accrued sick leave during the prior fiscal year. <br />Employees must have a minimum balance of 40 hours of sick leave to be eligible <br />for this benefit. <br />XXII. BEREAVEMENT LEAVE: Language in Article IV, Section 4 will be revised <br />as follows: "Up to 40 hours per occurrence, two occurrences each fiscal year, <br />may be taken by full-time employees with pay as bereavement leave." Use of <br />bereavement leave will include step parents and step children. <br />XXIII. INDUSTRIAL INJURY LEAVE INSURANCE PREMIUMS: Article IV, <br />Section 6, The City will pay the insurance premiums for employees on leave of <br />absence without pay due to industrial injury. Payment of insurance premiums <br />will include health, dental, life and optical. <br />XXIV. NEPOTISM POLICY: This policy applies to all City employees, appointed <br />officials and elected officials and includes adoptive children, son-in-law, <br />daughter-in-law, first cousin, niece, nephew, aunt, uncle and other relatives or <br />employees living in the same household. <br />
