ow <br />CONTRACTOR AGREEMENT <br />FOR <br />EMERGENCY MEDICAL SERVICES LIAISON <br />THIS AGREEMENT is made and executed as of November 1, 2007, by and <br />between the City of Colton (hereinafter "Agency') whose principal business is <br />650 North La Cadena, Colton, California 92324, and Loma Linda University <br />Medical Center, a California non-profit religious corporation which operates the <br />Center for Prehospital Care, Education and Research (hereinafter "Contractor"), <br />which has its principal place of business at 11155 Mountain View Avenue, Suite <br />220, Loma Linda, California 92354. <br />RECITALS <br />WHEREAS, Agency operates a fire department which includes EMT/paramedic <br />staffing; and <br />WHEREAS, Contractor wishes to assist Agency in the continued development, <br />training, and improved performance of its fire department by providing the <br />services of an Emergency Medical Services Liaison ("Contract Services") as set <br />forth in this Agreement; <br />NOW, THEREFORE, it is hereby agreed as follows: <br />ARTICLE I <br />SCOPE OF SERVICE <br />1.01 Scone of Services. Contractor shall provide the Contract Services set forth <br />in Exhibit A in accordance with the terms of this Agreement. <br />1.02 Availability. Contractor shall provide Contract Services on an "as needed" <br />basis, and at mutually agreeable dates and times. [Exhibit B sets forth the <br />maximum number of hours that Contractor is required to work during each <br />calendar month.] Agency shall provide a requested schedule of Contractor <br />Services at least two weeks prior to the scheduled time. Contractor shall <br />promptly notify Agency of any conflicts in the requested scheduling. <br />1.03 Contractor Qualifications. Contractor agrees to provide Agency with proof <br />of the following required qualifications for all persons providing services to <br />Agency pursuant to this Agreement: <br />1.03.1 Associates degree in Business, Health Care Administration, EMS, <br />or Nursing, at a minimum.
