A master copy of the workshop <br />packet will be sent to the sponsor for <br />duplication, three -hole punching and insertion <br />into the binders. The local sponsor is <br />responsible for the duplication costs of the <br />packet. <br />EVALUATION FORMS <br />A master copy of the NSS evaluation form will be <br />sent along with the workshop packet for <br />duplication. We ask for your help by requesting <br />that course participants complete and return the <br />form at the end of the program. The sponsor <br />agrees to compile a summary of the forms to send <br />to the attention of Susan Ivey as soon as possible <br />following the conference. <br />AUDIOVISUAL REQUIREMENTS <br />The local sponsor agrees to provide, lease and <br />operate the necessary audiovisual equipment <br />requested in advance by the faculty for both the <br />public and professional lectures. <br />VIDEOTAPING <br />Any videotaping of presentations by Dr. Brazelton <br />must be cleared in writing through the National <br />Seminar Series office, and may only be permitted <br />when used solely for educational or archival <br />purposes. The videotape(s) cannot be reproduced, <br />distributed or sold in part or whole. <br />SPONSORSHIP AND LAGOS <br />1.Sponsorship - The First Years is the "exclusive <br />national sponsor" for the NSS. Your organization <br />is considered the "local sponsor." <br />2.Logo - The local sponsor will recognize The <br />First Years by using their logo in all promotional <br />materials and whenever sponsors are mentioned or <br />listed.
