Beautification Sign <br />The Agency may place a 2'X3' sign in homeowner's front yard during the duration of the grant <br />work. Once the work has been completed, the Agency will remove the sign. <br />Reimbursement of Funds <br />Once work has been completed, Homeowner shall submit a Request for Reimbursement form, <br />along with copies of invoices or receipts to the Agency. Submission of the request shall indicate <br />that the Homeowner has accepted the work of the contractor (if any). <br />After conducting a 'windshield survey" to verify that the improvements have been completed, the <br />Agency will: 1) issue a two -parry check, made payable �o the Homeowner and contractor; 2) <br />reimburse Homeowner directly if payment has already been made to the contractor; or, 3) <br />reimburse the Homeowner directly if the Homeowner completed the improvements himself. The <br />Agency will not issue a check to the Homeowner unless it has received documentation that the <br />contractor, if any, has been paid. The Homeowner shall allow 3 weeks for the Agency to receive <br />a check. The Agency will not conduct an inspection ne be responsible for the workmanship or <br />warranties of the improvements. <br />The Agency will only reimburse funds for improvements that have been completed and will only <br />reimburse 66.7% of the request, up to a maximum of the Approved Grant Amount; the <br />Homeowner must fund at least 33.3% of the Approved Improvements. <br />Work completed without prior written authorization from the Agency will not be eligible for <br />reimbursement under the Grant program. <br />I have read the Rancho Mill Beautification Grant Program Guidelines and understand <br />the process. I understand that if I plan to deviate from the Approved Improvements <br />or extend beyond the expiration date, I must receive prior written authorization from <br />the Agency. I have been advised that these funds may be taxable and it's been <br />recommended that I consult with my tax advisor prior to requesting reimbursement. <br />I also understand that should I have any questions or require further clarification I <br />should contact the Agency at (909) 370-5167. <br />Homeowner <br />Date <br />
