CITY OF COLTON <br />650 NORTH LA CADENA DRIVE <br />COLTON, CA 92324 <br />CHILD CARE SERVICES AGREEMENT <br />This agreement Is made on 19 <br />eteen <br />City of Colton, Iocated at 650 North La Cadena Drive, Colton, bCaw <br />92324he <br />(hereafter, "City") and <br />(Address) <br />(hereafter, "Provider"). (Phone), <br />The parties recite that: <br />A. City is a contracting agency authorized to operate Child Care <br />Services to support the provision of Child Care and Development <br />Services Act, California Education Code Section 8200 et seq. <br />City Is the prime contractor to the State Department of <br />Education, Office of Child Development Services, Project No. <br />36-H129-01-03252-7. <br />B. Provider is an authorized subcontractor qualified to provide <br />direct care, supervision, and guidance for children for any part <br />of a day less than twenty-four (24) hours. Provider is acting in <br />an independent capacity and not as officers, or employees or <br />agents of City or State of California. <br />In consideration of the mutual promises hereafter contained, it is agreed <br />as follows: <br />PROGRAM RESPONSIBILITIES: City shall have the following responsibilities: <br />1. City shall render technical assistance concerning this Agreement. <br />2. City shall receive monthly claims from Provider, review said claims <br />for accuracy and program compliance „ and monthly, remit payment of <br />claim to Provider by mail delivery to the Provider's principal <br />address. <br />PROVIDER'S RESPONSIBILITIES: The Provider shall have the following <br />responsibilities, as a condition of payment under the contract: <br />1. The Provider shall furnish child care and supervision for children <br />under the age of 14 years who have been certified by the City as <br />eligible. <br />2. <br />The status of the Provider under this Agreement shall be that of <br />Independent contractor and at no time shall the Provider be or <br />represent hImself/herself to be an agent of the City.. <br />an. <br />
