This contract is entered into in the State of California by and between Los Padrinos of Southern California, Inc..; hereafter call.Contractor and City of Colton 650 North La Cadena Drive Colton, CA 92324 (714) 370-5086 The parties hereby agree as follows: hereafter called Sub -Contractor Article 1: Statement of Work This program requires the sub -contractor to perform at least the following tasks: 1. Provide immediate counseling to youth ages 8 - 17 years to include crisis counseling and resource and referral (new cases) to at least 125 youth, including those referred by the Probation Department. The program will provide follow-up and supportive services to an overall 200 youth with a minimum of 25 in the 13 - 17 age group. Intake will be reflected in the Sub -contractors monthly report to the contractor identifying clients by name and date of birth for the reporting month. 2. One youth counselor will,mintain an active caseload of 20 clients with a minimumof one meaningful face-to-face contact per client per month. In addition, 25 clients will be involved in a group counseling component during the program year. 8 - 10 youth will participate per quarter with a minimam of 2 contacts per month.
