a. A statement that the action is taken pursuant to the <br />Cortese/Knox Local Government Reorganization Act (Govt. <br />Code Section 56000); <br />b. The type of change being acted upon (annexation, <br />reorganization, etc.); <br />C. A description of the boundaries of the proposed change; <br />d. The LAFCO conditions for the proposal must be included in <br />their entirety; <br />e. The reasons for the change; <br />f. A statement regarding the use of assessment rolls; and, <br />g. A statement regarding assumption of contractual or bonded <br />indebtedness obligations of the jurisdiction by the <br />annexed area. <br />If any of these items are missing from the final resolution, <br />the LAFCO office will notify your jurisdiction and request a <br />corrected resolution. This will postpone the completion date <br />of the action. It may be of assistance to forward a draft copy <br />of the resolution to the LAFCO office for advance review to <br />preclude any problems which may affect the completion of the <br />proposal. <br />3. The completion package to be submitted to the LAFCO office for <br />processing must include the following: <br />a. Four certified and one non -certified copies of the <br />resolution of approval; <br />b. A copy of the affidavit from the newspaper indicating the <br />publication date of the Notice of Hearing; <br />C. Six copies of the map and legal description of the area; <br />d. 6 copies of a map showing street numbers along with a <br />description of the ranges. <br />(1) Block numbers are no longer acceptable by the Local <br />Tax Unit of SBE. The range of addresses needs to <br />show the highest and lowest possible street address <br />within the proposal in the following manner: <br />Example: <br />Apple Street: 2301 thru 2499 odd <br />2300 thru 2498 even <br />(2) The existing city limits must be clearly identified <br />2 <br />
