Work Subcontracted <br />CONCRETE FOUNDATION CONSTRUCTION <br />AND GROUNDING <br />Name of Subcontractor <br />QVL CONSTRUCTION, INC. <br />The undersigned certifies that he is licensed to engage in construction <br />contracting by the State of California for the work under these contract <br />documents. <br />State license number 581288 <br />License classification C-10 <br />The undersigned declares that he has visited the site of the work and <br />familiarized himself with the conditions affecting the work. <br />Enclosed herewith is the required proposal guarantee which the undersigned <br />bidder agrees is to be forfeited to and become the property of the City of <br />Colton, Colton, California, should this Proposal be accepted and a contract be <br />awarded to him and should he fail to enter into a contract in the form <br />prescribed and to furnish the required bonds, but otherwise the aforesaid <br />proposal guarantee will be returned upon his signing the Contract Agreement <br />and delivering the approved bonds. <br />The undersigned hereby declares that only the persons or firms interested in <br />the proposal as principal or principals are named herein, and that no other <br />persons or firms than herein mentioned have any interest in this Proposal or <br />in the contract to be entered into; that this Proposal is made without con- <br />nection with any other person, company, or parties likewise submitting a bid <br />or proposal.; and that it is in all respects for and in good faith, without <br />collusion or fraud. <br />If this Proposal is accepted, the undersigned bidder agrees to submit com- <br />pliance submittals in accordance with the contract documents and to perform <br />the work in accordance with the specified schedule. The undersigned fully <br />understands that time is of the essence in the completion of the work.
