PROPOSAL <br />TO: CITY COUNCIL <br />CITY OF COLTON <br />COLTON, CALIFORNIA 92324 <br />The undersigned bidder hereby proposed to furnish all labor, <br />material, equipment, tools and services necessary to perform all <br />work required for the construction of pumping facilities for Well <br />No. 24 in accordance with the specifications, drawings and all <br />addenda issued by City. <br />Bidder agrees that, within 10 calendar days after receipt of con- <br />tract from City, he will execute contract in the required form, <br />of which the Notice Inviting Bids, Drawings and all Addenda <br />issued by City prior to the opening of proposals, are a part, and <br />will secure the required insurance and bonds and furnish the <br />required insurance certificates; and that upon failure to do so <br />within said time, then the proposal guarantee furnished by bidder <br />shall be forfeited to City as liquidated damages for such <br />failure; provided, that if bidder shall execute the contract, <br />secure the required insurance and bonds, and furnish the required <br />insurance certificates within said time, his check, if furnished, <br />shall be returned to him within 5 days thereafter, and the bid <br />bond, if furnished, shall become void. It is further understood <br />that this bid may not be withdrawn for a period of 45 days after <br />the date set for the opening thereof, unless otherwise required <br />by law. <br />In conformance with current statutory requirements of Sections <br />1860, et seq., of the California Labor Code, the undersigned con- <br />forms the following as his certification: <br />I am aware of the provisions of Section 3700 of the Labor <br />Code which requires every employer to be insured against <br />liability for worker's compensation or to undertake self- <br />insurance in accordance with the provisions of that code, <br />and I will comply with such provisions before commencing <br />the performance of the work of this contract. <br />Bidder further agrees to complete all work required under the <br />contract within the time stipulated in said specifications and to <br />accept in full payment therefore the price(s) named in the above- <br />mentioned bidding schedule(s). <br />Dated �� �. `� i `i Bidder r, L)"I C <br />/ I <br />By , <br />Title r <br />M <br />
