The Honorable Mayor and <br />Members of City Council <br />May 11, 1994 <br />requested a resignation from the City Manager or the City <br />Manager has submitted a resignation in lieu of being <br />terminated by the City Council. <br />2. Revise the City Manager Ordinance 2.16. 070 (B) to clarify <br />when City Council is actually ratifying the employment of <br />Department Heads, i.e., at the time of hire, prior to <br />passing the Department Head from probation to regular <br />status, or at both the time of hire and prior to passing <br />the Department Head from probation to regular status. <br />3. Create a policy to preclude employees who are separating <br />from employment with the City to take documents from City <br />premises without the approval of the City Attorney or the <br />Personnel Director. <br />4. Create a policy requiring employees to sign the Personnel <br />Action Record (PAR) whenever there is a change in the <br />status of employment record that would necessitate the <br />completion of a PAR and its placement in the employee's <br />official personnel record. In the event an employee <br />refuses to sign the PAR, a witness may write onto the <br />form that the employee refused to sign the PAR and sign <br />his or her name as having made the effort to collect the <br />employee's signature. <br />The transmittal of these findings and recommendations to the City <br />Council concludes the responsibilities of the Civil Service Board, <br />in the matter of Herron, as set forth in the above cited <br />Resolutions. <br />Respectfully submitted on this -/7 fa('., day of May, 1994. <br />T, <br />Yolanda Mope z _ U <br />Chairperson, Civil Service Board <br />2 <br />
