Section 4: <br />Optical Plan <br />The City shall provide an annual amount of up to $140 per <br />employee and $90 per eligible dependent towards a vision <br />examination, lenses and/or frames. <br />Section 5: <br />Term Life Insurance <br />The City shall provide a total of $50,000 term life insurance <br />for each represented employee effective the first of the month <br />following Council adoption of this MOU. Employees may purchase <br />employee supplemental or dependent life insurance coverage in <br />accordance with the terms made available by the City's insurance <br />provider. Said additional coverage shall be at the employee's sole <br />expense and shall be paid through authorized payroll deduction. <br />Section 6: Long Term Disability <br />The City shall provide to each represented employee a long <br />term disability program with an eligibility period of 30 days or <br />upon the expiration of sick leave, whichever is the greater. The <br />terms of the plan shall be more fully set forth in the plan <br />documents; however, effective the first of the month following City <br />Council adoption of this MOU, it shall provide for up to five years <br />of coverage at 66-2/3*-. of the first $6000 of the employee's base <br />salary, reduced by any deductible benefits. The elimination period <br />is defined as the first 30 calendar days of each period of total <br />disability. The employee may choose to supplement the disability <br />allowance with accumulated paid leave up to a maximum of 1001 of <br />base salary including the disability allowance. However, should <br />the employee elect to use sick leave, the equivalent dollar value <br />shall he deducted from the disability allowance. <br />Section 7: Tuition Reimbursement <br />The City agrees to reimburse employees up to $1,000 per <br />employee, per year, so long as funds are available, for tuition <br />costs incurred for job-related education. Such expenditure must <br />enhance furtherance of City or continuing educational goals. <br />Requests for such reimbursement must be approved by the Department <br />Head and Personnel Officer. Employee -initiated educational or area <br />development shall not be considered as time actually worked for <br />purposes of computing overtime and normally shall not occur during <br />regular work hours. <br />Section 8: <br />Uniforms <br />The City agrees to furnish uniforms (six shirts) and laundry <br />service for employees working on line or service crews (including <br />employees in the Utility Worker classification) .
